Lacticaseibacillus paracasei CNCM I-1518 is the strain deposited in the Danone-associated Institut Pasteur collection and used in fermented dairy products studied in the context of immune resilience. A 2020 systematic review and meta-analysis of RCTs examined a fermented milk containing this strain in relation to the incidence, duration, and severity of common infectious diseases. Additional RCTs have examined its use in children in day-care settings and its relationship to gut microbiome recovery following antibiotic or other drug therapy. Animal studies have examined proposed mechanisms including control of respiratory pathogens and Paneth cell activity in laboratory settings; Paneth cells are specialized intestinal cells that produce antimicrobial peptides. The human evidence base is among the more substantive in this genus, though most trials studied the strain as part of a dairy matrix rather than as an isolated supplement.
